This formulation contains Clopidogrel Bisulfate, a thienopyridine class antiplatelet agent.

• Mechanism (P2Y12 Inhibition): Clopidogrel is a prodrug. Once swallowed, it is metabolized by the liver (specifically CYP2C19 enzymes) into its active form. This active metabolite irreversibly binds to the P2Y12 class of ADP receptors on the platelet surface.
• The “Sticky” Blocker: By blocking these receptors, it prevents Adenosine Diphosphate (ADP) from activating the platelets. This stops platelets from clumping together to form dangerous blood clots (thrombi).
• Irreversible Action: The effect lasts for the entire lifespan of the platelet (7–10 days). Normal platelet function is restored only as new platelets are generated by the body.
• Stent Protection: It is the primary drug used (often with Aspirin) to prevent “Stent Thrombosis”—a fatal complication where a clot forms inside a newly placed cardiac stent.

• Polymorph Control: Clopidogrel Bisulfate exists in two crystal forms. Form I is kinetically unstable. We strictly utilize Polymorph Form II, which is thermodynamically stable, ensuring the drug does not degrade or change crystal structure during shelf storage.

Dosage & Administration

Recommended Dosage (Strictly as per Cardiologist):

• Route: Oral.
• Loading Dose (Emergency): 300 mg (4 tablets of 75mg or 1 tablet of 300mg) taken immediately upon admission for ACS.
• Maintenance Dose: 75 mg once daily.
• Combination: Often taken with low-dose Aspirin (Dual Antiplatelet Therapy – DAPT) for up to 12 months after stent placement.
• Timing: Can be taken with or without food.

Safety Warnings:

• Bleeding Risk: The most significant risk is bleeding. Discontinue 5–7 days before elective surgery to restore platelet function.
• PPI Interaction (CRITICAL): Omeprazole and Esomeprazole inhibit CYP2C19, the enzyme needed to activate Clopidogrel. Taking them together makes Clopidogrel ineffective. Pantoprazole is the preferred safe alternative.
• Poor Metabolizers: Some patients genetically lack CYP2C19 enzymes (“Non-responders”) and may require alternative drugs like Ticagrelor.
