- Heart Failure: Target dose is often higher, e.g., 75 mg four times daily combined with nitrates.
- Frequency: Hydralazine is a “fast metabolizer” (short half-life), requiring dosing 3 to 4 times a day for consistent effect.
Safety Warnings:
- Reflex Tachycardia: By rapidly dropping blood pressure, it can cause the heart to race dangerously. Almost always co-prescribed with a Beta-Blocker to prevent this.
- Drug-Induced Lupus: Long-term use at high doses (>200mg/day) can cause a lupus-like syndrome (joint pain, fever, rash). This is reversible upon stopping the drug. Acetylator status (genetic) affects this risk.
