Dosage & Administration
Recommended Dosage (Strictly as per Cardiologist):
- Route: Oral.
- Hypertension: Start 2.5 mg once daily. Titrate to 5 mg or 10 mg based on response.
- CV Risk Reduction (HOPE Protocol): Start 2.5 mg daily for 1 week, then 5 mg for 3 weeks, then increase to 10 mg once daily (Target Dose).
- Post-MI / Heart Failure: Start very low (1.25 mg or 2.5 mg twice daily). Titrate to target 5 mg twice daily.
- Administration: Swallow whole with liquid. Can be taken with or without food.
Safety Warnings:
- Dry Cough: A persistent, dry, hacking cough is a common class effect of ACE inhibitors (due to bradykinin accumulation). If intolerable, switch to an ARB.
- Pregnancy (Black Box Warning): Strictly Contraindicated. Causes fetal injury and death (oligohydramnios). Stop immediately if pregnancy is detected.
- Hyperkalemia: Increases potassium levels. Monitor electrolytes, especially if taking potassium supplements or Spironolactone.
- Angioedema: Rare but fatal swelling of the face/throat. Emergency attention required.
